The National Care centre has a commitment to ensuring that our clients and their families have the highest quality care and support.We will visit and conduct a face-to-face assessment to understand all of your requirements.If you’re looking for a provider for a loved one, or thinking about one for yourself, it’s a big decision – so it’s important to find the care that feels just right.

You have the freedom and flexibility to choose the schedule and the services you want. Since everyone’s care needs are different, the Aged Care Assessment Team (ACAT) grants packages on four different funding levels of care:

The Range of Care and Services Available Under a Home Care Package Includes, But is Not Limited to:

Help with Household Tasks

We will help you keep your home clean and liveable.

  • Help with making beds
  • Help with ironing and laundry
  • Help with cleaning like dusting, vacuuming, and mopping
  • Help with unaccompanied shopping
  • Minor garden maintenance including weeding, pruning, and lawn mowing.
  • Nutrition, Hydration, meal preparation & diet 
  • Assistance with preparing meals, assistance with using eating utensils and assistance with feeding

Equipment Assistance

We will help you with mobility, communication, reading, and personal care limitations.

  • Walking aids like crutches, quadruped walkers, walking frames, walking sticks
  • Mechanical devices for lifting you in and out of bed
  • Bed rails
  • Aids like slide sheets, sheepskins, tri-pillows
  • Pressure-relieving mattresses

Minor Home Modifications

This is a type of help-at-home service. We will help you live safely and independently in your home.

  • Installing easy access taps
  • Installing grab rails in the toilet, bath and/or shower
  • Installing a ramp
  • Providing advice on areas of concern regarding the safety of your home
  • Fixing uneven flooring
  • Cleaning gutters
  • Accessing technical assistance for major home modifications (that are not included in My Aged Care services)

Personal Care

We will help you maintain your personal hygiene and grooming standards.

  • Help with bathing, showering, toileting
  • Help with dressing/undressing
  • Assistance with getting in and out of bed
  • Help with washing and drying hair, and shaving
  • Reminders to take your medication
  • Continence management: assistance in using continence aids such as disposable pads and absorbent aids, commode chairs, bedpans and urinals,  catheter and urinary drainage appliances and enemas

Clinical Care

We will help you treat and monitor medical conditions at home.

  • Wound care and management
  • Someone to help you take your medication
  • General health and other assessments
  • General health and treatment education to improve self-management
  • Allied health and physiotherapy services
  • Nursing, Allied health and therapy: speech therapy, podiatry, occupational, physiotherapy service 
  • Clinical services: hearing and vision services

Set Up Your Home Care Package with Us

Our relationship-based approach to care means that we make an effort to understand the individual needs of each person we care for. We take the time to assist them in choosing the right type of services to suit their needs, and we ensure that they receive the best value for their care.

  • Choose the support services you’d like
  • Choose your support worker and set the schedule
  • Agree on how much the support workers are paid
  • Get more hours of support and make the most of your package
  • Develop personal connections 

What Services are NOT Included in Home Care Packages?

Although you have choices when selecting the services you need and the provider you prefer, there are some services that don’t qualify for funding from your Home Care Package. 

Generally, anything that you would normally purchase out of your income doesn’t qualify for funding. These are…

  • Grocery purchases (but special food purchases for enteral feeding requirements are included)
  • Paying your mortgage or rent (but home modifications that are necessary for accessibility likely do)
  • Entertainment activities such as club memberships or tickets to events aren’t eligible for funding, but activities that help you stay connected in your community may be. 
  • Holiday trips, but a health-related retreat could be included. 
  • Any fees or charges that are already funded by the Australian Government (such as services covered by the Medicare Benefits Schedule)
